Care Plans

Within each resident's EMR, you can create a comprehensive Care Plan.

 

To assist you, we provide a Problem, Goal and Approach (PGA) Library developed with input from our client facilities. The library also includes problems, goals and approaches written in “I Care Plan” language. Edit this library to meet the needs of your residents.

 

Our library-based care plan systems allow you to:

  • Select and edit frequently-used text rather than re-describe similar needs.
  • Link Care Areas (CAs) to specific problems that address the CAs.
  • Perform Quality Assurance to locate all residents who have a specific problem, goal and/or approach in their care plans.

The Care Plan module offers two Care Planning Methods: Manual and CAA-driven.
The CAA-driven method suggests problems that will address the CAAs.
Both formats allow you to incorporate the following in your residents’ Care Plans:

  • Problem types of Active, Maintenance, Potential, Resolved and Inactive.
  • Various sort numbers that allow you to sort care plans various ways--by problem number, problem category, or by resident’s primary needs.
  • Target dates on goals.
  • Discipline codes that identify staff who will carry out the approach.
    • Print Care Plan responsibilities by discipline.
    • Include Nurses Aides’ approaches on residents’ monthly Flow Sheet records.

Printed Care Plan records include:

  • The quarterly Care Plan with signature page, in portrait or landscape mode. 
  • Monthly Care Plan Evaluation for documenting goal progress for up to three months. Use this tool to prepare for goal evaluations each quarter.
  • Goal Reviews that locate goals with target dates that should be met.
  • Problem histories.
