The following long term care software products help our clients to assess and document residents’ care and conditions and to assist in other facility operations, such as Billing and Human Resources.
Hi-Tech Clinical Assessments Can Reveal Risk Levels and Trends
Clinical Assessments provide a library of assessment areas that includes:
|
Risk for Falls |
Risk for Dehydration |
Skin Risk |
|
Contracture Risk |
Nutritional Risk |
Bowel/Bladder Training |
|
|
|
- Each area includes a set of criteria and a standard set of responses, and each response has a score.
- When you assess a resident for a specific area, the program adds the scores of all responses to calculate a total score.
- Compare the total score to the score range that indicates level of risk/acuity.
- The program maintains a history of assessments. Printed assessment can include up to five assessments, allowing you to look for trends and changes.
- You can create your own assessments and edit those provided by Hi-Tech.
- To begin using this feature, from the Clinical Records System choose Documentation > Clinical Assessments. Consult the Clinical User Manual for instructions.
Click Clinical Assessments (pdf) to see a sample Risk for Falls assessment.
MDS Quality Assurance Reporting
The MDS Quality Assurance Report allows you to choose up to nine MDS items to compare or contrast their responses. This is a very powerful tool that can be used to track potential problems such as residents who have fallen and who also take antipsychotic medications.
- From Clinical: Nursing Care choose QA / QI / Survey Reports > Quality Assurance > MDS.
- At Census Date you have three options:
- Current date--to search the most recent MDS for each resident.
- Change the date to search each resident's most recent MDS as of that date.
- Enter a date range of MDS records that you want to search.
- At Section, display and select from the list of MDS sections, questions and the responses. Click Add to Criteria List. The cursor will return to Section so you can choose another question.
- Finally, click either Must Meet All or Any of the criteria chosen. The report will list residents who meet the selected criteria. It will display the date of the MDS used and MDS responses.
MDS and RCA Data Logic Checking Detect Logic Errors
Review MDS responses that might conflict with each other even though CMS will accept the responses. You might discover inconsistencies that would be questioned by your surveyors. Or you might identify areas to reconsider and report more accurately to raise your case mix index.
The program checks for more than 120 conflicting responses, and provides a variety of reporting formats. You can check open, completed, and submitted assessments, and print or display the results in detail or summary formats .

See the Guest Commentary: Will you find the Discrepancies in your MDS records before the Surveyors Do? published in the Florida Health Care Association May 2006 issue of Pulse Magazine.
We also offer MDS-RCA Data Logic Checking for Residential Care/Assisted Living, which reviews MDS-RCA records (for Maine Residential Care facilities) for 70 possible conflicts in responses.
Increase your Reimbursement
The Rehab Therapy System is designed for therapists to record therapy details immediately after a session with a resident. The system interfaces with the Resident Billing system to automatically create the charge records, which can be reviewed at month-end before posting. It also reports therapy minutes for Medicare A residents and recommends Optimum MDS start dates for reporting the greatest amount of therapy in the MDS. Recorded therapy can automatically complete the MDS Therapy items.
Use Human Resources System to Track Employees
The Human Resources System allows you to record pre-screening and interview details and then transfer this information into the employee record when the applicant is hired. You can access the employee's complete profile and history from one location. Human Resources will integrate with the Hi-Tech Payroll System. More about Human Resources (pdf)
